At Advantx Home Care patients are evaluated not only on the disease process, but as a holistic being. The admitting RN evaluates the patient's living environment, financial factors, and caregiver availability. The nurse acts as a facilitator to set up resources available to the client and assist them in returning to a higher level of independence.
The Home Care Team, along with the physician, will develop a plan of care with measurable outcomes throughout the Home Care 60 day episode. Each episode is monitored by the physicians through Care Plan Oversight which is billable by the physician.

In home skilled nurse assessment of cardiovascular and circulatory status using state of the art portable 12 lead EKG and Doppler devices producing immediate results for physician review and response.
